- Agreement to Terms
These Terms & Conditions (“Terms”) are a legally binding agreement between you and Vetted Healthcare Group LLC (“Company,” “Vetted Healthcare,” “we,” “us,” or “our”), governing your access to and use of our website and services.
BY ACCESSING OR USING OUR WEBSITE OR SERVICES, YOU CONFIRM THAT YOU HAVE READ, UNDERSTOOD, AND AGREE TO BE BOUND BY THESE TERMS. IF YOU DO NOT AGREE, DO NOT USE OUR WEBSITE OR SERVICES.
- Company Overview and Services
Vetted Healthcare Group LLC, operating as Vetted Healthcare, is a Wisconsin-based healthcare staffing company headquartered at 11512 North Port Washington Road, Suite 201C, Mequon, Wisconsin 53092. We provide staffing services to healthcare facilities across Wisconsin and connect qualified healthcare professionals with shift and placement opportunities throughout the state.
2.1 Staffing Services for Healthcare Facilities
- Long-Term Care Staffing — skilled nursing facilities, assisted living, memory care communities, post-acute and rehabilitation units, Residential Care Apartment Complexes (RCACs), community-based residential facilities (CBRFs), and adult family homes.
- Hospital and Outpatient Staffing — hospitals, Critical Access Hospitals, urgent care centers, ambulatory surgery centers, outpatient clinics, and specialized healthcare settings.
- Home Care and Home Health Agency Staffing — nurse and caregiver staffing for Wisconsin home care and home health agencies.
- Flexible Staffing Models — per diem, travel, short-term and long-term contracts, temp-to-hire, and permanent placement.
2.2 Opportunities for Healthcare Professionals
We connect registered nurses (RNs), licensed practical nurses (LPNs), certified nursing assistants (CNAs), allied health professionals, caregivers (including CBRF-trained caregivers), and non-clinical support staff with opportunities at Wisconsin healthcare organizations. Professionals may apply through the Quick Apply form at vettedhealthcare.com/quick-apply.
2.3 Career Advancement
Vetted Healthcare operates a voluntary workforce development initiative that connects candidates with advancement opportunities and partner-supported training options when available, and places them in healthcare settings that fit their goals. Participation is voluntary. Any referral to a training partner is made with the candidate’s consent and does not create a contractual obligation between the candidate and the training partner through us. We make no representation regarding any training program’s outcomes, costs, or results.
- Eligibility
3.1 General
You must be at least 18 years of age, have the legal capacity to enter binding agreements, and not be prohibited from using our services under applicable law.
3.2 Healthcare Professional Eligibility
To register and receive active placements, you must:
- Hold a current, active, and unrestricted professional license or certification in your discipline;
- For CNAs: hold active, clear status on the Wisconsin Nurse Aide Registry with no substantiated adverse findings;
- For RNs and LPNs: hold an active Wisconsin license verified through the Wisconsin Department of Safety and Professional Services (DSPS);
- Be legally authorized to work in the United States;
- Have no active exclusions from Medicare, Medicaid, or any federal healthcare program (OIG/SAM exclusion check required);
- Pass required background checks; and
- Meet all additional credentialing requirements specific to your role and specialty.
3.3 Healthcare Facility Eligibility
To submit staffing requests and receive services, your facility must:
- Be a licensed healthcare organization in good standing in the State of Wisconsin;
- Hold appropriate facility licenses and accreditations required by Wisconsin law;
- Carry adequate professional liability and general liability insurance;
- Be represented by an authorized individual who can legally bind the organization; and
- Not be excluded from participation in Medicare, Medicaid, or other federal healthcare programs.
- Accounts and Registration
4.1 Account Accuracy
You agree to provide accurate, current, and complete information and to update it promptly. Providing false information is grounds for immediate termination and may expose you to legal liability.
4.2 Account Security
You are responsible for maintaining the confidentiality of your account credentials and for all activity under your account. Notify us immediately at support@vettedhealthcare.com of any unauthorized access.
4.3 Account Termination
We reserve the right to suspend or terminate accounts that violate these Terms, provide false information, engage in prohibited conduct, or remain inactive for extended periods. We will provide notice where reasonably practicable.
- Terms for Healthcare Professionals
Applies to RNs, LPNs, CNAs, allied health professionals, caregivers, and all professionals who apply through Vetted Healthcare.
5.1 Accuracy of Credentials
You represent and warrant that all professional information you provide is accurate, complete, and current — including licenses, registry status, certifications, training, employment history, malpractice history, and any disciplinary actions. You have an ongoing duty to update this information without delay if anything changes.
5.2 Wisconsin License and Registry Requirements
- RNs and LPNs: maintain an active, unrestricted Wisconsin license verified through the Wisconsin DSPS. Notify us immediately of any restriction, suspension, or disciplinary action.
- CNAs: maintain active, clear status on the Wisconsin Nurse Aide Registry. Notify us immediately of any adverse notation, registry suspension, or finding of abuse, neglect, or misappropriation.
- All credentials: you are responsible for renewing licenses, certifications, and registry status before expiration. Failure to maintain active credentials terminates your eligibility for placement.
5.3 Compliance During Assignments
When placed at a Wisconsin healthcare facility through Vetted Healthcare, you agree to:
- Comply with all applicable federal, Wisconsin state, and local laws and regulations;
- Follow all facility policies, protocols, and procedures from your first shift;
- Maintain patient confidentiality and comply with HIPAA requirements at the facility;
- Adhere to professional standards of conduct and applicable licensing-board ethical guidelines;
- Practice only within your scope of licensure and training — do not accept assignments requiring competencies you do not hold;
- Report workplace incidents, errors, safety concerns, or patient harm immediately through appropriate facility channels;
- Complete all required documentation accurately and within required timeframes; and
- For long-term care placements: follow applicable staffing-documentation and facility-specific charting protocols.
5.4 Background Checks and Credentialing Consent
By applying and proceeding through credentialing, you consent to:
- National criminal background check;
- Sex offender registry search;
- OIG/SAM federal exclusion list screening;
- Wisconsin DSPS license verification (primary source);
- Wisconsin Nurse Aide Registry verification (for CNAs);
- Drug screening as required by specific facility placements;
- A minimum of three professional reference checks; and
- Ongoing monitoring of license and registry status during active placement periods.
Unsatisfactory results on any of the above may disqualify you from active placement.
5.5 Reliability and Attendance
Healthcare facilities depend on confirmed staffing. You agree to arrive on time for all confirmed shifts; notify us and the facility as early as possible if you cannot fulfill a confirmed shift; and understand that repeated last-minute cancellations or no-shows may result in suspension or permanent removal.
5.6 Non-Solicitation
You agree not to directly solicit or accept direct employment from any healthcare facility introduced to you through Vetted Healthcare within twelve (12) months following the conclusion of any placement at that facility, without our prior written consent. Violation may result in liquidated damages as specified in your individual placement agreement.
5.7 Compensation and Payment
- Pay rates are established in individual assignment agreements and confirmed before shift acceptance;
- Payment is processed weekly via direct deposit for completed, confirmed shifts;
- Your employment classification determines tax withholding obligations; and
- Compensation disputes must be reported within 30 days of the relevant pay period by contacting support@vettedhealthcare.com.
5.8 Career Advancement Program
Participation in our career advancement and training-partner referral program is entirely voluntary and does not affect your ability to access shifts. Training-program referrals connect you with independent third-party organizations; Vetted Healthcare makes no representations about those programs’ outcomes, costs, or results.
- Terms for Healthcare Facilities
Applies to SNFs, ALFs, memory care, post-acute, RCACs, CBRFs, adult family homes, hospitals, CAHs, urgent care, ASCs, clinics, home care and home health agencies, and all healthcare organizations.
6.1 Authority to Contract
By submitting a staffing request or entering into a service agreement, you represent that you are authorized to bind your organization to these Terms and any applicable service agreements.
6.2 Accurate Facility Information
You represent that all facility information provided is accurate, including facility name, license type and number, accreditation status, contact information, billing details, and an accurate description of staffing needs including required credentials, experience level, shift type, and urgency.
6.3 Facility Environment and Safety
You agree to:
- Provide a safe working environment compliant with OSHA and applicable Wisconsin workplace-safety standards;
- Provide all equipment, supplies, and access required for the placed professional to perform their duties;
- Provide appropriate orientation for each placed professional before their first patient-care responsibility;
- Ensure appropriate clinical supervision and oversight consistent with your scope of services; and
- Treat placed professionals with respect and professionalism consistent with your obligations as a healthcare workplace.
6.4 Regulatory Compliance
You remain solely responsible for your facility’s compliance with all applicable regulatory requirements, including any CMS minimum-staffing requirements for skilled nursing facilities. Vetted Healthcare provides staffing support and placement documentation; we do not guarantee that our placements will bring your facility into compliance with any specific regulatory threshold. Compliance responsibility remains with you.
6.5 Timekeeping and Approvals
You agree to verify and approve hours worked accurately and within required timeframes, dispute any timesheet discrepancies within five (5) business days of submission, and not alter timesheet records without documented agreement from all parties.
6.6 Payment Terms
- Invoices are payable within the terms specified in your service agreement (typically Net 15 or Net 30);
- Late payments may incur interest at 1.5% per month (18% annually) or the maximum rate permitted by Wisconsin law;
- We reserve the right to suspend services for accounts more than 30 days past due; and
- Emergency and short-notice placements may require pre-payment authorization.
6.7 No Direct Hire Without Conversion Fee
You agree not to directly hire any professional placed through Vetted Healthcare for a period of twelve (12) months following the conclusion of any placement, without our prior written consent and payment of a conversion fee as specified in your service agreement. This applies to all placements, including per diem, short-term contract, and temp-to-perm arrangements.
6.8 Quality Concerns and Replacements
- Notify your account contact within 24 hours of any quality, performance, or safety concern involving a placed professional;
- We will address documented concerns promptly and arrange replacement coverage when warranted;
- We are not responsible for clinical decisions made by placed professionals acting under your supervision and direction; and
- Concerns must be documented in writing for investigation and quality-improvement purposes.
6.9 Emergency Staffing Requests
For emergency staffing requests (coverage needed within 24 hours), facilities should call (888) 817-8233 directly rather than submitting an online request. We target a response to all urgent requests within 2 hours during business hours.
- Intellectual Property
7.1 Our Property
The following are owned by Vetted Healthcare Group LLC and protected by applicable intellectual property laws: the names Vetted Healthcare and Vetted Healthcare Group and all associated logos, trademarks, and service marks; all website content, design, and technology; and all written content, staffing-documentation templates, and marketing materials.
7.2 Limited License to Users
We grant you a limited, non-exclusive, non-transferable license to access and use our website for its intended purpose. You may not copy, reproduce, or distribute our content; reverse engineer our systems; use our names or trademarks without written authorization; or scrape or frame our website.
7.3 Your Submitted Content
By submitting professional profiles, feedback, or other content, you grant us a non-exclusive, royalty-free license to use that content to operate and improve our services.
- Prohibited Conduct
You agree not to:
- Provide false, inaccurate, or misleading information about your credentials, facility, or identity;
- Impersonate any person, professional, or organization;
- Submit credential documentation that has been altered, forged, or misrepresented;
- Violate any applicable federal, Wisconsin state, or local law or regulation;
- Harass, threaten, or harm other users, placed professionals, facility staff, or patients;
- Attempt to gain unauthorized access to our systems or other users’ accounts;
- Use our website to compete with, replicate, or undermine our services;
- Transmit viruses, malware, or any harmful code;
- Circumvent credentialing, background-check, or registry-verification requirements;
- Engage in discriminatory conduct based on any protected characteristic under applicable law; or
- Use our website for any purpose other than legitimate healthcare staffing.
- Disclaimer of Warranties
TO THE MAXIMUM EXTENT PERMITTED BY WISCONSIN AND FEDERAL LAW, OUR WEBSITE AND SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT. WE DO NOT WARRANT THAT PLACED PROFESSIONALS WILL MEET SPECIFIC CLINICAL EXPECTATIONS, THAT PLACEMENTS WILL BRING FACILITIES INTO REGULATORY COMPLIANCE, THAT QUALIFIED PROFESSIONALS WILL BE AVAILABLE FOR ALL SPECIALTIES AND SHIFTS, OR THAT OUR WEBSITE WILL OPERATE WITHOUT INTERRUPTION OR ERROR.
- Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, VETTED HEALTHCARE GROUP LLC SHALL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, OR CONSEQUENTIAL DAMAGES; LOSS OF REVENUE, BUSINESS, OR CENSUS; PATIENT OUTCOMES OR ADVERSE CLINICAL EVENTS; REGULATORY SURVEY DEFICIENCIES OR CIVIL MONETARY PENALTIES; CLINICAL DECISIONS MADE BY PLACED PROFESSIONALS ACTING UNDER FACILITY SUPERVISION; OR DAMAGES EXCEEDING THE TOTAL FEES PAID TO VETTED HEALTHCARE GROUP LLC IN THE THREE (3) MONTHS PRECEDING THE CLAIM. THIS LIMITATION APPLIES REGARDLESS OF THE LEGAL THEORY ASSERTED AND EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
Some jurisdictions limit the exclusion of certain damages. In such jurisdictions, our liability is limited to the maximum extent permitted by applicable law.
- Indemnification
You agree to defend, indemnify, and hold harmless Vetted Healthcare Group LLC, its members, officers, employees, and agents from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys’ fees) arising from: your use of our services; your violation of these Terms; your violation of any applicable law or professional licensing requirement; for clinicians, professional misconduct, license violations, credential misrepresentation, or patient harm resulting from your clinical actions; for facilities, clinical incidents involving placed professionals acting under your supervision, failure to provide safe working conditions, or unauthorized timekeeping alterations; and any content you submit.
- Professional Liability and Clinical Responsibility
12.1 Professional Liability Coverage
Details of any professional liability insurance coverage provided by Vetted Healthcare for active placements are specified in individual placement agreements. Coverage details, limits, and tail-coverage arrangements vary by placement and will be disclosed before any assignment begins.
12.2 Facility Clinical Responsibility
Healthcare facilities retain clinical responsibility and liability for the care delivered within their facilities, including the supervision and direction of placed professionals. Vetted Healthcare is a staffing intermediary — we are not a healthcare provider and do not exercise clinical judgment or direct patient-care decisions.
12.3 Clinician Professional Responsibility
Placed professionals are responsible for practicing within their scope of licensure and training, regardless of facility requests. No instruction from a facility or from Vetted Healthcare overrides your professional licensing obligations or state scope-of-practice requirements.
12.4 No Medical Advice
Vetted Healthcare does not provide medical advice, clinical guidance, or patient-care recommendations. We are a healthcare staffing company, not a healthcare provider.
- Dispute Resolution
13.1 Informal Resolution First
Before initiating formal proceedings, contact us at support@vettedhealthcare.com to attempt informal resolution. We will respond within 10 business days and make a good-faith effort to resolve the dispute.
13.2 Binding Arbitration
If informal resolution fails, disputes shall be resolved through binding arbitration administered by the American Arbitration Association (AAA) under its Commercial Arbitration Rules, in Milwaukee, Wisconsin (or virtually if agreed), before one neutral arbitrator. The decision is final and binding.
13.3 Class Action Waiver
YOU AND VETTED HEALTHCARE GROUP LLC EXPRESSLY WAIVE ANY RIGHT TO PARTICIPATE IN A CLASS ACTION LAWSUIT OR CLASS-WIDE ARBITRATION. ALL DISPUTES MUST BE RESOLVED INDIVIDUALLY.
13.4 Exceptions
Either party may seek injunctive relief in court for intellectual property violations. Small claims court jurisdiction is preserved for eligible claims.
13.5 Governing Law
These Terms are governed by the laws of the State of Wisconsin, without regard to conflict-of-law principles. Any court proceedings shall be filed in Milwaukee County, Wisconsin.
- Equal Opportunity and Non-Discrimination
Vetted Healthcare Group LLC is an equal opportunity organization. We do not discriminate in our staffing, placement, or service operations on the basis of race, color, national origin, religion, sex, gender identity or expression, sexual orientation, age, disability, veteran or military status, or any other characteristic protected by applicable federal or Wisconsin law. Facilities partnering with us agree to accept placed professionals consistent with all applicable equal employment opportunity laws and without unlawful discrimination.
- Privacy
Your use of our website and services is governed by our Privacy Policy, available at vettedhealthcare.com/privacy-policy, which is incorporated into these Terms by reference.
- Electronic Communications
By providing your contact information and using our services, you consent to receive electronic communications related to your account, placements, and applications; operational notices including placement confirmations, credential reminders, and billing communications; and, where you submit an urgent or emergency staffing request, phone or SMS contact from our team for coordination. You may opt out of marketing communications at any time. You cannot opt out of transactional communications related to active placements, compliance matters, or credential-status changes.
- Force Majeure
Vetted Healthcare Group LLC is not liable for failure to perform obligations caused by circumstances beyond our reasonable control, including natural disasters, pandemics or public-health emergencies, government mandates or regulatory changes affecting healthcare staffing, technology-infrastructure failures, or other events outside our control. We will make reasonable efforts to identify alternative coverage and notify affected parties promptly.
- Severability
If any provision of these Terms is found to be unenforceable or invalid under applicable law, that provision will be modified to the minimum extent necessary to make it enforceable, and all remaining Terms remain in full force and effect.
- Entire Agreement
These Terms, together with our Privacy Policy and any applicable individual placement agreements or facility service agreements, constitute the entire agreement between you and Vetted Healthcare Group LLC regarding your use of our services, and supersede all prior agreements and understandings on these subjects.
- Modifications to Terms
We may update these Terms as our services evolve. We will update the “Last Updated” date and, for material changes, notify registered users by email. Your continued use of our services after changes are posted constitutes acceptance. If you do not accept updated Terms, discontinue use and notify us at support@vettedhealthcare.com.
